ECM Advocate is built on one clear priority: Patient Care First. Since 2024, we have delivered high-quality Enhanced Care Management services across Southern California with a focus on compassion, accountability, and meaningful outcomes.
Founded by a group of healthcare leaders with more than 75 years of industry experience, ECM Advocate brings deep expertise in population health, readmission reduction, home health capitation, and consulting across multiple healthcare business models.

Enhanced Care Management provides person-centered care coordination for individuals with complex medical, behavioral, and social needs. We help assess needs, develop individualized care plans, connect clients to providers and community resources, and support ongoing follow-up to improve health and stability.

Our Community Health Workers help bridge the gap between clients and the services they need by providing outreach, education, advocacy, and resource connection. They work closely with clients, families, providers, and community partners to promote engagement, build trust, and support better outcomes.

Housing Navigation supports individuals experiencing housing instability by helping them identify barriers, connect to housing resources, and work toward housing access, placement, and stability. Services include coordination with landlords, shelters, providers, health plans, and community organizations to support safe and sustainable housing solutions.
Community Health Workers at the Core
Community Health Worker services are one of the core services of ECM Advocate and a key part of the foundation our platform was built on.
Our CHWs help members navigate healthcare systems with confidence by building trust, improving communication, supporting follow-up, and connecting individuals and families to essential community-based resources.
